Astrofest Waiver and Release of Liability
By checking the box below, you acknowledge and agree to the following terms as a condition of membership and participation in any activities sponsored by Chiefland Astro Inc. (Chiefland Astronomy Heritage Museum):
1. Assumption of Risk: I understand that participating in astronomy-related activities—including public observing sessions, telescope use, educational programs, and outreach events—may involve inherent risks. I voluntarily assume all such risks.
2. Release of Liability: I release and hold harmless Chiefland Astro Inc., its officers, directors, volunteers, and members from any and all claims, liabilities, or causes of action arising out of my participation or use of facilities and equipment.
3. Medical Emergencies: I authorize Chiefland Astro Inc. to obtain emergency medical care on my behalf if I am injured or become ill during an event, and I accept financial responsibility for any such care.
4. Property Damage: I agree to be responsible for any damage I cause to property or equipment during activities, whether accidental or intentional.
5. Photography Release: I grant permission for photos or videos taken during events to be used for promotional, educational, or documentary purposes by Chiefland Astro Inc., unless I notify the organization in writing to opt out.
6. Minors: If I bring any minors to Chiefland Astro Inc. events or facilities, I accept full responsibility for their supervision and safety, and I agree to these terms on their behalf.
